Advocates for Access & Accountability (AAA) is an independent public-interest initiative focused on documenting systemic patterns, risks and governance gaps in publicly funded disability care. AAA does not provide legal, medical, or investigative determinations and does not replace the role of regulators, law-enforcement agencies, courts or statutory oversight bodies.

Information shared with AAA is collected to support pattern identification, policy analysis, transparency, and evidence-informed advocacy. Submissions reflect the experiences of individuals, families and legal decision-makers are treated as reported accounts unless corroborated through multiple sources or publicly available documentation. AAA does not adjudicate disputes, determine fault or make findings of legal or regulatory non-compliance.

References to neglect, abuse, access restriction, injury, or rights violations are used descriptively to identify systemic risk and conditions of harm. They are not statements of criminal, civil or regulatory liability.

Any transparency tools, summaries, or comparative frameworks developed by AAA are intended to support informed decision-making, prevention of harm and public accountability. They do not constitute endorsements, certifications, compliance ratings or guarantees of care quality and should not be relied upon as the sole basis for placement decisions, legal action or regulatory assessment.

AAA is committed to ethical data stewardship, confidentiality and the protection of vulnerable individuals. Identifying information is not published without explicit consent and sensitive information is handled in accordance with applicable privacy principles, ethical standards and harm-reduction practices.

Participation in AAA initiatives is voluntary. Where immediate health, safety or rights concerns exist, individuals are encouraged to seek appropriate legal, medical or emergency support.
