Care Planning & Supports

Care planning is a core obligation within publicly funded systems of care. It is not an administrative formality, nor is it a discretionary exercise controlled solely by service providers. Ethical, lawful care planning requires meaningful participation, transparent documentation and ongoing accountability, including the active involvement of the individual, families and legal authority in care.


This section outlines how care planning is intended to function, where it commonly breaks down and how families can navigate planning, reviews and transitions when systems become exclusionary, delayed or opaque.


AAA provides structured care-planning, documentation and advocacy support to help families prepare, participate and protect their role in publicly funded care processes. 


AAA does not replace service providers; rather, it supports clarity, alignment and accountability when planning processes fail to meet their obligations.  It is to share information and learnings as much as its s to foster care and collaboration which is, lead by the individual, family and that of the legal authority in care.

This service supports families and substitute decision-makers in clarifying care needs, identifying gaps or risks, and establishing a clear advocacy foundation when care systems are not meeting obligations. The session focuses on:


  • Reviewing the current care situation and priorities.
  • Identifying unmet needs, risks, or breakdowns in care.
  • Clarifying roles, responsibilities, and expectations.
  • Establishing an advocacy focus and practical next steps.


This service is foundational and often the first point of engagement.  It is  advisory and preparatory in nature. It does not include clinical assessment, legal advice, representation, or formal care planning by regulated professionals.

This service supports families and advocates in aligning individual care experiences with broader advocacy, public accountability and government-facing strategies when systems fail to meet their legal, ethical, or regulatory responsibilities.


The focus is on moving beyond internal documentation toward strategic readiness, ensuring experiences are framed, supported and positioned in ways that can be meaningfully engaged by regulators, oversight bodies, ministries, and public accountability channels. The session supports:


  • Positioning individual care experiences within systemic and regulatory contexts.
  • Identifying responsible authorities (service providers, funders, regulators, ministries).
  • Preparing documentation for accountability, escalation, or public-facing use.
  • Strengthening advocacy narratives to support oversight, reform and results.


This service is appropriate when families are preparing to escalate concerns or engage beyond the service-provider level. It is strategic and advisory in nature. It does not include legal advice, political lobbying, media representation or advocacy conducted on behalf of the family.

This service supports families managing home-based or community care in coordinating schedules, roles, communication and collaboration among caregivers, support workers and service providers.


Drawing on lived experience within the home-care and PSW sector, this service focuses on practical coordination strategies that reduce conflict, improve communication and support sustainable care environments particularly during transitions.  The session focuses on:


  • Structuring care coordination and routines.
  • Clarifying roles and expectations across caregivers
  • Supporting transitions in care settings or staffing.
  • Strengthening communication and collaboration to meet support expectation in care and home plans.
  • Foster opportunities in scheduling to build on family, fairness and community connections. 


This service provides coaching and systems guidance only. It does not include clinical supervision, employer direction or workforce management.

Empowering our Community with Advocates for Access & Accountablity

AAA affirms that collaboration in care is a shared, ethical and accountable process, not a discretionary courtesy extended by service providers. Meaningful collaboration requires transparency, timely information-sharing, and the inclusion of the individual and their legal authority in care in all planning, review and transition processes.


AAA rejects models of “collaboration” that:


  • Exclude families or legal authority in care from planning or reviews.
  • Withhold or delay critical information
  • Substitute procedural compliance for meaningful engagement.
  • Frame disagreement or advocacy as non-cooperation.


True collaboration respects the rights, roles, and responsibilities of all parties. It recognizes that families and legal decision-makers are not peripheral participants, but essential contributors to ethical, person-centred care.


Where collaboration is compromised, AAA supports families and legal authorities in documenting concerns, preparing for engagement and asserting their role within care planning and accountability processes in a structured, professional and rights-aligned manner.
